Intermittent login failure using correct credentials

Apologies if this has been covered before, I've searched the forums with no success so far.

Every now and again I am blocked from logging in to SF- the password field is auto-populated by my browser (Firefox), and 95 times out of 100 this works, but occasionally I get a login failure.

Whenever this happens, I paste the password into the required field, to be sure that there are no typos, but the failure persists. Usually after some random interval I can log-in again, with exactly the same credentials.

Has anyone else encountered this? It's frustrating in the extreme when it occurs.

Do you use more than one Salesforce org?  I find that this happens to me sometimes when switching between orgs.  The way I resolve it is by clearing my login cookies.
